Alcohol and Sleep Aids: A Synergy With Serious Consequences

Prescription sleep medications—benzodiazepines, non-benzodiazepine hypnotics like zolpidem—are also among the most commonly dispensed drug classes in the country.

Alcohol is a central nervous system depressant. So are both categories of sleep aids. When combined, the sedative effects do not simply add together—they potentiate each other, meaning the combined impact exceeds the sum of the individual parts. The resulting over-sedation can suppress the respiratory drive, particularly during deep sleep, creating a genuine risk of respiratory depression. By accelerating CYP3A4 activity, St. John's Wort speeds the breakdown of any medication processed through this pathway—which includes a remarkable range of drugs: oral contraceptives, antiretrovirals, certain antidepressants, immunosuppressants used in organ transplant patients, and several cancer medications. A transplant patient who begins taking St. John's Wort without disclosing it to their care team may find their immunosuppressant levels dropping to sub-therapeutic concentrations, raising the risk of organ rejection.

For patients on oral contraceptives, this interaction is particularly consequential and particularly underreported. Studies have documented cases of unintended pregnancy linked to St. John's Wort use in women who were not informed of the interaction.
