The problem is that each checkpoint can fail independently. Physicians work under time constraints. Pharmacy software has known limitations in capturing the full clinical picture. Patients, especially those managing multiple conditions, are not always equipped to evaluate clinical risk on their own.

This is precisely why the pharmacist's verification role deserves more public understanding than it typically receives. It is not a formality. It is not a liability exercise. It is a trained professional applying clinical knowledge to a real patient at a critical moment in their care.
